Gemish Gives $$ Value over C&S
While ear canal cytology is recommended for every case, for most episodes of
otitis externa/media, bacterial culture and susceptibility testing has limited
clinical value, in terms of antibiotic selection. This is true for a number
of reasons. Many laboratories do not report minimum inhibitory concentrations
(MICs); those that do use MICs test the antibiotics at levels far lower (mcg)
than can be delivered topically in the "Gemish 2" formulation (see the 7 Steps
handout at New 7 Steps Handout In this
formulation, the antibiotic is being delivered at 10 MG/ml (not mcg). MIC values
are established for parenteral administration, not topical administration. Further,
the recommended volume (filling the ear canal) using the "Gemish 2" formula
is sufficient to effectively reach and cover most, if not all, of the affected
surface area in the ear. Finally, a number of studies have drawn into question
the reliability of C & S results taken from cases of canine otitis externa.
Most alternatives involve using potentially ototoxic antibiotics. Taken together,
this leads us to the conclusion that the use of the "Gemish 2" formula based
on cytology results is a prudent choice for pseudomonal otitis externa/media
with a much lower cost:benefit ratio than ear cultures.

Industry Summit, Portland
For the 7th year, an exclusive, invited group of the world's leaders of veterinary industry met in Portland at a summit hosted by Banfield. The golf tournament commenced the meeting and, like in life, perennial winner (and host), JOHN PAYNE's team won. Our team of RICHARD NORRIS, JIM CLEARY and RYAN HAEMMER took unheralded 3rd. DAVID WALDRUP, BOB ROHDE, MARK CUSHING and JONATHAN BETTS nosed our team out for 2nd place. What does someone (WALDRUP) from the FDA having a 5 handicap say about government? WALDRUP, asked to do his job, was notified that CLEARY, with mysterious power, appears to be dipping into the growth hormone or steroid stock at MWI and/or is slipping away from work early to practice his golf swing. Although no one was there from the SEC, BRUCE BERNING advised us all early of what we shouldn't be (heard) talking about. JIM NAVE , who seems to be everywhere (in just the last 30 days, seen in New Orleans and Vancouver), MARTY BECKER, GUY PIDGEON attended as did the leaders of almost all leading companies. The BANFIELD CHARITABLE TRUST once again had a successful auction led by SANDY CAMPBELL. No wonder one of the leading donors was SCOTT CAMPBELL.
Key bits of info: Banfield has 720 hospitals now, expected to grow to 750 by the end of the year. 53% of vets in US have Banfield employment experience.2400 vets graduate each year. They will hire 600 vet in 2008, all of whom will attend an academy ( the next one will be the 162 organized by the able TIM MOONEY). There are 72.1 and 81.7 million dogs and cats, respectively. The average vet sees only 10 cases/day. And, my personal favorite from JOE BIELITZKI, Illini grad, consultant to NASA "-40 degrees is where Fahrenheit and Centigrade come together". He reminded us that "risk is an essential component to progress." He also added that "many find problems to solutions, though, few find solutions to problems." Most of the people at this summit represented the latter.
