Virtual Reality Misinformation

Many of you may have noticed that for the first time DermaPet is trying to sell products using conventional sales techniques as opposed to strictly on the merits of our products. Credit should go to Gary and Jack for placing these programs in place. Having Gary and Jack around has also afforded us with "ears on the street" with regards to what other companies are saying and doing.

It appears some competitive companies' sales people have located a large population of vets unfamiliar with cyber-jargon. To this unsuspecting group, these unscrupulous sales people are twisting our claim as "the largest vet derm company in virtual reality" saying that, of course, DermaPet is not as big as DVM or Virbac. They then infer that if we are misleading about this then "what can DermaPet be trusted to say?" For the sake of the minority of you who read this Internet only letter, I will define "virtual reality" as "in cyberspace, the internet or the world wide web." To that end, using that definition, DermaPet is clearly the leader in veterinary dermatology in "virtual reality". Consequently, in "reality", we have removed the "cyber-phrase" from as much print advertising as possible and placing "#1 Web Site in Veterinary Dermatology." "Heard on the Street"

This is a section that we want to relate stories related by practitioners using our products. If requested, your name, practice and location will appear. This months experience is from a practice that sold the DentAcetic to a dentist, although skeptical about other benefits, had a relationship with his pet that became "closer" after using DentAcetic as a "breath freshener."

Ototoxicity /Chlorhexidine

Contrary to rumors well supported by marketers of ototoxic products, there are choices to treat ears that do not involve the use of ototoxic chemicals. This newsletter will discuss one of the worst. It is a fact that chlorhexidine has been shown to cross even intact tympanum and cause permanent pathological otic damage. (Mansfield). It is also a fact that no product containing any active amount of chlorhexidine is labeled or 'publicly' marketed for use in ears.

Companies that label a product as a "Flush" containing chlorhexidine can not place on the label or market to vets either directly or indirectly that product for use in ears. An animal that suffers an adverse reaction using one of these products leaves the owner with the right to claim liability on the part of all selling parties. An overused explanation is that the pet was already suffering from progressive ear problems. Several cases have been reported and have been well demonstrated on the Internet. It is always best to use products that are labeled for the target organ you are attempting to treat.

Ed note: We have received numerous calls and comments from vets about the above paragraph. It is still not well known that chlorhexidine has no place in otic therapy. Please feel free to contact me about your experiences involving any derm or otologic experience, in particular, the use of ototoxic substances.

Malassezia and Bacterial Otitis PROTOCOLS

While it is true that the acetic/boric acid products have efficacy against Pseudomonas, they are actually FIRST LINE topical treatment while the TrizEDTA (Step 6) remains the best protocol for treatment after the acetic/boric acid is used using Step 3b or Step 4 (See the Seven Steps to Treating Ears under Protocols at http://www.dermapet.com). BOTH OF THESE PRODUCTS CAN BE USED IF THERE IS A RUPTURED TYMPANUM.

RECENT RESEARCH by Dr. Louis GOTTHELF and SULIMAN GBADAMOSI (Tuskeghee, Micro Dept) indicate that there is a need to instill TrizEDTA, preferably by a cleaning exercise, before using antibiotics. In fact, when testing samples that were previously reported as resistant to flouroquinolones (using MICs), eight dilutions of our stock TrizEDTA were required to not inhibit growth. This indicates that the TrizEDTA is bactericidal by itself.

Step 6.

1.Orbafloxin (Dicural/Ft Dodge) Marbofloxin (Zenoquin/Pfizer) or Baytril (Bayer) systemically for at least 21-60 days.

For those of you wanting to know how much Baytril to give a patient orally, you need to know the MIC of the organism. The rule of thumb is that you want a tissue level of Baytril to be 8 times the MIC. You can find tissue levels in the package insert for Baytril. Based on the tissue level, you can figure out how many mg/kg you will need to administer to your patient. Are you OK with 5mg/kg or do you need to use 20mg/kg?

Most labs do not give you this information, only if it is sensitive, intermediate, or resistant. If you want to know the endpoint MIC for your intermediate or resistant Pseudomonas, please send the specimen and a check for $12.00 to Dr. Sulaiman Gbadamosi, Tuskegee University, School of Veterinary Medicine, Tuskegee, Alabama 36088. He will do the endpoint MIC for you.

2.Clean with TrizEDTA twice daily.

3.Approximately 15 minutes after each cleaning, instill Baytril (or another appropriate antibiotic (12 ml of saline and 50-200 mg of Baytril injectable) or diluted with saline. Some people use aminoglycosides even though they are ototoxic.

4. For the first 14 days, I would use systemic prednisone at 1 mg/kg IF OM is diagnosed. The rationale is to decrease the inflammation, mucous secretion from the mucoperiosteum, pain and viscosity of the exudate in the tympanic cavity.

5.If OM is diagnosed, I would anesthetize and, with visualization, flush the ear with TrizEDTA and directly instill Baytril and NaDex phosphate into the tympanic bulla. If th tympanum is present and OM is suspected then I would do a myringotomy. The location of the myringotomy should be ventral. Maintenance therapy would include biweekly TrizEDTA cleansing longterm. Some people recommend our acetic/boric acid Ear/Skin Cleanser instead since it has activity against Pseudomonas as well as Malassezia.

Remember: Do NOT be misled by various forms of misinformation as to why certain companies do not use boric acid with acetic acid. The reality is that DermaPet holds a patent on this combination.
