Geriatric Otology/ Protocol for Otitis Media Excerpt from
Geriatrics & Gerontology of the Dog
GERIATRICS & GERONTOLOGY of the Dog and Cat, 2nd Ed., JOHNNY
HOSKINS, published by Elsevier, just came including my chapter Ear Diseases
and Altered Hearing.
The following is an excerpt from Geriatric Otology/ Protocol for Otitis Media.
1. Systemic antibiotics such as Baytril or Marbofloxin for 21 days. This has
been reduced in importance to emphasize topical therapy.
2. Clean with TrizEDTA twice daily
3. Approximately 15 minutes after each cleaning, instill Baytril in either a
TrizEDTA gemish (12 ml of TrizEDTA and 50-200 mg of Baytril injectable) or diluted
with saline . Some people use aminoglycosides even though they are ototoxic.
Ed note: since writing the chapter, I prefer to combine 2 and 3 by adding 6
ml of LA Baytril (600 mg) to a 4 ounce bottle of TrizEDTA and instilling .5-2ml/bid/each
ear.
4. For the first 14 days, I would use systemic prednisone at 1-2 mg/kg if OM
is diagnosed. The rationale is to decrease the inflammation and mucous secretion
from the mucoperiosteum, pain and viscosity of the exudate in the tympanic cavity.
5. If OM is diagnosed, I would anesthetize and, with visualization, flush the ear with TrizEDTA and directly instill Baytril and NaDex phosphate into the tympanic bulla. If the tympanum is present and OM is suspected then I would do a myringotomy. The location of the myringotomy should be ventral. Maintenance therapy would include biweekly TrizEDTA cleansing longterm.
US Senate Passes the National Veterinary Medical Service Act
The US Senate passed the National Veterinary Medical Service Act on Nov. 24.
Earlier in the month, the House of Representatives unanimously passed the bill,
which authorizes the USDA to provide student loan repayments to recent veterinary
graduates who agree to work in underserved areas of the country. President Bush
signed the bill December 6.
European Commission Proposes Issuing Wallet-sized Passports for Dogs, Cats and Ferrets
The European Commission proposed issuing wallet-sized passports for dogs, cats and ferrets in an effort to make it easier for pets to travel with their owners in Europe. The owners would be able to use a standardized blue booklet listing vaccinations and certifying that the pet is rabies-free. The passports would identify animals by microchip, tattoo, or an optional photograph. The document would replace the current pet travel papers issued nationally by the 15 EU governments. Veterinarians would be responsible for issuing and detailing each animal's medical history in the passport.

miles and they snickered thinking it was cute, but, how would she use the benefits?
United has bankruptcy logic, what with the majority of their 3 cabin 1st class
seats filled with their own pilots doing business while the cash
paying business class passengers can not upgrade to the pilot
cabin. Maybe some pet loving airline will give passport carrying, microchip
wearing (my Roxie is afraid to get a tattoo!) pets a program they deserve.
VOLUME Is Very Important In Ear Treatments
VOLUME is very important in ear treatments. For the treatment of bacterial otitis,
while previous newsletters addressed the tweaking of the Gemish
(as described in 7 Steps to Ear Therapy, at www.dermapet.com ), it is important
to emphasize that .5-2 ml of Gemish/liquid should be applied TWO times daily
in each affected ear. This pretty much eliminates all commercial products, which
generally come in 7.5 to 15 ml (large) sizes.
When treating Pseudomonas or other bad microbes, the current recommendation
is to add 6 ml (600 mg) of Baytril LA (Large Animal injectable) or Marbofloxin
100 mg/ml, if you are in a place where you can get it. If systemic corticosteroids
are not used then, for the first 2 weeks, take out 24 ml aliquots and add 8
mg of Dexamethasone Sodium Phosphate (DSP). Then clean the ear twice daily with
TrizEDTA and treat with the DSP Gemish.
The volume is required to reach the large affected surface area in the external
horizontal canal and the tympanic bulla in the middle ear. The importance for
large doses of systemic antibiotics on a long term basis has been downplayed
(by other experts) to only 21 days (by yours truly); others suggest using even
less systemic antibiotics. Topical treatment, on the other hand, could last
for months.

It snowed again! In fact, the Williamsburg Chamber of Commerce is considering
asking the Lees (RALPH, MARTHA and HOWIE) to change their show to Xmas
Day so they can be assured of a white Xmas. Lots of people on diets there. RON
WHITFORD, my Atkins mentor is down over 40 pounds. Even Martha LEE has lost
substantial weight. LOU GOTTHELF made it in late and back to ATLANTA just in
time for a Medrx seminar on Sunday! CHUCK MICHAELS manned the booth.
From Down Under, JIM ADAMS of Deltavet, our distributor there, tells me that
the Australian Open will be played at the Moonah Links, the site of our golf
outing. Speaking of golf, the big golf tournament is 1/17 in Orlando at the
Marriott, sponsored by news, magazine and now meeting conglomerate, DVM. In
case you didnt know, they now own the CVC in KC and some magazines including
the well read Vet Economics. Its getting more difficult for small pharmaceutical
companies like DermaPet to play those advertisers against each other.
Speaking of geriatrics and hearing loss, I went to a wonderful nostalgic, performance
by Simon and Garfunkel in DC which was well attended by the over 40 crowd. You
have to be that old to know that one word as a career (Plastics)
from The Graduate which this generation presumes has something to
do with appearance enhancement.
