Total Ear Canal Ablation

Sometimes our best protocols (using TrizEDTA, of course) to treat Otitis Media fail and we need to do surgery. The following is from the Carol Foil's article on Otitis Media at www.dermapet.com.
Total Ear Canal Ablation (TECA) with Bulla Osteotomy This is reserved for end-stage ear disease when the health and function of the horizontal canal cannot be restored. It is desirable to avoid this drastic surgery, but in my opinion all dogs with chronic otitis media (OM) should have this surgery rather than live with the disease. If we can extrapolate from human experience, OM is a painful condition. Dogs with unresolved OM are subject to several sources of misery: Pain on opening their mouths, continuous pain from pressure on the tympanum until it ruptures, deep infection of the soft tissue of the horizontal canal. This occasionally results in draining tracts opening on the lateral neck, side of the face. Neurological consequences are discussed in the article.

Complications of TECA: Should be discussed thoroughly before referral. Hearing loss-Most dogs with end stage otitis externa have significantly decreased hearing before surgery, and studies using brain stem auditory evoked responses have shown that hearing is usually minimally affected by the surgery. The improvement in quality of life resulting from pain relief far outweighs the significance of any marginal loss of hearing.

Animal ID

The USDA announced it is holding a series of listening sessions across the country to discuss development, structure, and implementation of a national animal identification program for all livestock and poultry animals. Last year, you read right here that we suggested Passports for all domestic animals...and a frequent flyer card for Roxy, my Yorkie whose picture is in our ad in DVM Magazine this month. Only, the good people at United didn't get the humor of it. One pilot gave her wings after a particularly rough flight where after getting sick and cleaned up in the bathroom she fell into the toilet and almost fell into someone's Midwestern pasture. I've been making it up to her ever since.

JACK WHITE, CHUCK MICHAELS and I ran the booth. A special guest appearance from ROXY, our mascot Yorkie, ran afoul of the law when GLORIA was asked to leave the trade show hall. Ever so cute Roxie can be viewed in our August DVM magazine ad.

Our first night we ate at Ralph's Italian with GARY and SUSAN LANDSBERG, DEBORAH HOROWITZ and ILANA REISNER. The real entertainment was on the side where quick-quipping ILANA and former rodeo cowboy Jack sat. At one point, after Jack overheard Ilana was from PENN, asked, in his best Texas drawl, "Yur-at-Penn?" to which Ilana acted confused (thinking Jack said "European") quipped, "No, I'm from the United States." Gary nearly fell out of his chair. And Gary, incoming ACVB President, (and Susan) had been sitting for a long time since they had to drive 9 hours from Toronto to Philadelphia due to weather canceling their flight. Jack just took it in stride.

VLS gave us a plaque awarding us the most informative ad in our class. STEPHEN WHITE, MITCH SONG and ROD ROSYCHUK were on the program that, to DermaPet's liking, included a 1/2 day on Otology. One session by a prominent derm was on "How to clean an ear" and another was on what product to use. I applaud the inclusion of such basic practice and hope people can visit the DermaPet sponsored Otology seminar at the NAVC in Orlando, 2005 to see more of it.

DON KLNGBORG, UC/Davis Assoc Dean for CE was on hand to proudly introduce his son, JOHN KLINGBORG, as the President of the California VMA. RON WHITFORD presented business practices. COLIN BURROWS and GENERAL RICHARD FORD were seen around. The General was seen brunching at the Four Seasons with a former student who must be a spy.

Welcome to Philadelphia, city of brotherly love. Poor CHRIS SCOTT (Dan Scott Co) had his car broken into and lost his booth. Even worse, DRAKE SNEAD of Intervet had some booth material in the car as well.

In support of the use of DENTEES STARS, PETER EMILY, at his dental wet lab at the SEVC in Myrtle Beach, noted that that 38% of dogs were less than 20 pounds and that these tiny pets represent the majority of dental practice. He gave the following explanation.

Once bacteria gets within 1/2 mm of bone in the gingival sulcus you get active bone resorption which continues until bone density is 1.5-2 mm thick. Continued bone loss leads to tooth mobility and subsequent tooth loss. The loss of the bone further reduces the resistance to the advance of bacteria. They then can be seen to have reduced bone density, which, of course, may lead to further complications including loss of the tooth. Further, these pets seem to live longer than their larger counterparts thus having even more disproportional bone resorption. He further explained that this bone resorption is from a byproduct of neutrophil metabolism, usually IL-1 and prostaglandins, which are catalytic to bone. (ed note: Conclusion: Feed your dogs and cats DENTEES to prevent dental disease.)
