MalAcetic HC Prototol

There have been many questions on when or how to use the MalAcetic HC. Since it is virtually 50% strength MalAcetic Otic with hydrocortisone added, logically one would use it on inflamed, irritated, erythematous, edematous, (perhaps) ulcerated skin first. After a few to several days on the MalAcetic HC switch to regular strength MalAcetic.

Some experts recommend using systemic corticosteroids alone with no topicals in severely inflamed otitis cases. This will allow the ear to "cool down" prior to placing any topicals, almost all of which could be potentially irritating. After this "cool down" then start the MalAcetic HC for about a week or less followed by the regular MalAcetic Otic.

MalAcetic HC is not a replacement for MalAcetic Otic, which should remain the veterinarian's first choice to clean an ear, treat an ear with Malassezia otitis (see articles by Gotthelf or Bassett or Benson at the web site) or prevent ear disease. Also remember, up until now we have not had a case of ototoxicity reported due to MalAcetic Otic, so it is safe for cases where there is a ruptured tympanic membrane.
Because MalAcetic contains both acetic and boric acids which each and combined have other significant antimicrobial effects (see Benson's article), we also place them in topicals such as shampoo, spray-on leave-on conditioners and wet wipes where they can help fight Staphylococcus as well as Malassezia.

Below is a repeat from last months newsletter which supports use of the MalAcetic HC logic.
The notes from DermaPet's 1st Annual OTOLOGY SYMPOSIUM at TNAVC are posted at our web site (with permission from TNAVC). Below are excerpts from this presentation from LOU GOTTHELF.

Corticosteroids have a definite place in the treatment of otitis externa. .....1% hydrocortisone (MALACETIC HC, DermaPet) may be used in the ear to act as a preventative for inflammation in atopic dogs that may have recurrent otitis. Corticosteroids do not remove hyperplastic epithelium or glands, so if there is no response to high dose corticosteroids after 7-10 days, the stenosis is probably the result of increased tissue growth rather than inflammation. ...

For otitis externa complicated by Malassezia, the author prefers the use of an acetic acid/boric acid solution (MALACETIC OTIC). The cleansing effect and desquamating effect of the acetic acid/boric acid solution essentially removes fatty acid substrates necessary for the metabolism and reproduction of Malassezia. Malassezia produces a chemotactic factor for neutrophils that is hydrophilic and protein in nature. The presence of this factor may help explain why only a few Malassezia organisms can cause such profound erythema and pruritus. The cleansing effect of the acetic acid/boric acid solution destroys this acid-labile chemoattractant and may account for the reduction in inflammation. Boric acid, which is hygroscopic (drying out the humid ear canal) removes moisture necessary for this hydrophilic chemoattractant and may prevent attachment of Malassezia yeasts to the epithelium by inactivating adhesin molecules that allow Malassezia to stick to keratinocytes.

MAL ACETIC OTIC is used daily for 7-10 days by filling the ear canal to overflowing, massaging the base of the ear, and allowing the solution to remain in the ear canal for 5 minutes. The loosened debris is wiped off of the pinnal surface with a dry cotton ball. This procedure is repeated once daily. When the ear canal is clean, the cotton ball will remain fairly white when the solution is wiped away. At that time, treatment is reduced to once weekly.

The golf tournament went well. LOU GOTTHELF won a closest to the hole contest while JAY LOCKHART actually aced the 13th hole. The ANTECH team won it this time (YT et al, deciding to rest on our TNAVC winning laurels, could not make it).

We enjoyed the annual Chinese seafood feast at the Joyful House with LOU and PENNY GOTTHELF, JACK WHITE, PATRICK RAYBURN, JOYCE HEFLIN, KIM CAREY, ALAN GARETT, GLORIA and YT attending.

JOHN ERHARDT (UI '83) came by the booth and remembered me from my teaching days at ILLINOIS.. The WVC staffs were quite responsive to exhibitors needs this year. When a suggestion to STEVEN CRANE was made to place a sitting area near the wall by our booths, it was done moments later. And, ROXY, the DermaPet mascot, received her first show badge after being pampered with attention at the Exhibitor sign-in desk, run by VICKI CRANE, making me realize I should have brought her years ago. If only we could communicate as well with their other committees (i.e., Program).
It was good to have actress/model STACEY MELMAN who showed up on Monday at the DermaPet booth attracting many visitors although few who were our target audience.
The unusual rainy weather was good for a trade show.

Certain other "news" (i.e., select items about the colorful Medrx team) have been omitted due to the "what happens in Vegas, stays in Vegas" rule.

PETER EMILY and BOB WIGGS lectured. WIGGS suggests further coating our DENTEES with other meds he recommends to increase exposure time on the teeth and gums of dogs.

PENN, Philly
What a treat it is to see my alma mater and so many classmates at this meeting! They finally made (read "were forced to make") the much-needed change to a downtown hotel. Classmates BOB CUSANO, JAY LEEB, DON FORD, JAMES HYSONG, VERN and Elaine DURIE all said hello. DON MARCUS, my classmate all the way back to my days at TEXAS A&M, ANDY BRESLIN and HYSONG and I reminisced over a few drinks. Another ILLINOIS student, AMY KIESCHE NESSLERODT visited. Potomac neighbor, RICHARD WEITZMAN, is the current Maryland State Golf Association President. How you can play golf that well and practice to the level of excellence, that only a neighbor would know he does, is amazing. Much to the amazement of DEAN ALAN KELLY, his son, IAN KELLY, courteously checked me in and was fascinated that I knew his dad socially way back then. The prices at JAY GERSON's practice just went up as his daughter just got accepted to next year's class. I recognized the tears of joy in his eyes. Delmarva may be practicing reverse age discrimination with CHUCK BETTYMEN, BOB CASTAGNA and JOE KUHTA all apparent (not to the eye) septuagenarians.

AAHA, Baltimore
I lunched with DONNA GINDY and COLIN BURROWS. Congratulations go to Colin who received the Royal K9 Award for International Contributions to Veterinary Medicine. DAN NEGOLA and BILLY AMOROSO are building a big kennel in the outskirts of DC. STEVE ROGERS, who used to work for my father, is the current Secretary/Treasurer of AAHA. Based on my experience working with him, AAHA has the right person. Neighbor NORM ROSKIN conversed with me about our local emergency clinic partnership situation. I lunched with DEB HOROWITZ and GARY LANDSBERG although Deb's lunch coupon appeared to be a counterfeit, xeroxed off an original. CHRIS VON STEIN, (COO), DAVE BROWN (CFO), GLORIA and YT, JACK WHITE and JIM ADAMS (our Australia distributor) all helped celebrate CHUCK MICHAELS birthday in Little Italy. Jim Adams traveled a long way to see the NCAA playoffs and learn some more details about promoting our products Down Under.

I invite you to read all of the above articles at our web site. In particular, note that MalAcetic is 87% and 88% effective against Malassezia and, at .5% MalAcetic is effective against Staphylococcus while at 1% is effective against Pseudomonas. Note: that the 2% strength provided by our MalAcetic Otic and the rest of the MalAcetic line is what you have been relying upon. Now, for those sensitive, inflamed, pruritic or erythematous cases, you can use a gentler product with hydrocortisone.
