Dermatophyte Tx Failures

Dermatophytosis/ LimePlus Dip
LimePlus Dip has increased the awareness of the use of Lime sulfur for use as
the best, and perhaps, only treatment for Demodex gatoi as well as the topical
treatment of choice for dermatophytosis. Still there are many treatment failures
as described in this article from Moriello (which appears at our web site).
In my experience, the most common causes of treatment failure are an incorrect
diagnosis. This most commonly occurs when clinical signs alone are used to make
a diagnosis, underscoring the need to definitively diagnose dermatophytosis
based on a positive fungal culture result alone or in addition to a positive
result on direct microscopic examination of hairs.
Reinfection
This most commonly occurs when a cat is exposed to a contaminated environment
or to other cats with positive culture results, such as those that are sub clinically
infected or carriers.
Environmental decontamination is extremely important in the treatment process,
not unlike treating the environment in flea infestation cases. And the more
animals involved in the outbreak, the greater the environmental contamination.
A contaminated environment. This can cause consistently positive culture results or fluctuations between negative and positive culture results, especially if only a few colonies are present on the fungal culture plate.
Resistant organisms
It is unknown how often resistance occurs. Most of the cases of resistance
I have investigated on referral have been related to owner compliance problems;
incorrect drug doses; drug intolerance by a cat; the inability to medicate cat;
or exposure to carrier cats, contaminated environments, or persistently infected
cats. Each of these causes, with the exception of drug intolerance, should be
investigated before making changes in antifungal drug therapy. In cases of drug
intolerance, consider changing to a divided-dose schedule or a different drug.
Concurrent illness, certain medications, or a defective immune system. Concurrent
systemic illnesses are usually more obvious than defective immune systems. Common
concurrent illnesses or therapies that can complicate resolution of dermatophytosis
include hyperthyroidism, diabetes mellitus, chronic renal failure, neoplasia
or chemotherapy, glucocorticoid administration, demodicosis, and infection with
feline leukemia or feline immunodeficiency virus.
Refusal to clip medium-haired or longhaired cats or any cat with generalized
dermatophytosis. Unfortunately, many owners become convinced of the need to
clip their cats' coats only after treatment takes longer than expected or infection
persists.

cures. It is true that while most tourists will be looking at examples of ancient
civilization or shopping for souvenirs, YT can be found in local apothecaries
and herbal remedy shops. First dog,Roxy, will not be taking this trip, although
she certainly would like to and I have no fear of animal right abuses.

PEROXIDE, 1% SULFUR and 1% Salicylic acids. It is the only benzoyl peroxide
product containing both sulfur and salicylic acids on the market. Demand for
this product has been increasing since a similar product without salicylic acid

MalAcetic HC Wipes are particularly useful to treat skin fold problems, especially
in inflamed areas. The most common of these include breed predispositions such
as bulldogs, most spaniels, Bassett’s, Shar pei as well as vulvar, facial, eye,
chin, lip and tail folds. Cats with eosinophilic granuloma complex, particularly
the eosinophilic ulcer and dogs with “hot spots” aka ‘pyotraumatic dermatitis’
do well on these since they have both antimicrobial as well as anti-inflammatory
effects.
Reports are in that TrizEDTA Plus is enjoying growing support as a flushing
agent for tracheal washes as well bacterial rhinitis. Experts are adding 1000-1200
mg of aminoglycosides such as amikacin or gentocin to the 4-ounce bottle before

flush for FUS and other resistant cystitis cases.
Yada, Yada

We will be at the ACVC in Atlantic City on October 17-19 as well as many regional and state meetings. I will be lecturing on Otology in SEOUL on October 22, in WUHAN at the CHINA Small Animal Veterinary Association (CSAVA) annual meeting on October 29 then BEIJING on October 31.
We have been advised that the Mandalay Bay and The Hotel in Las Vegas have
a severe room shortage for Sunday, February 18. In fact, there are no rooms
available that night due to the NBA All Star game. My advice is to book your
rooms NOW at the Four Seasons and other nearby hotels while you can get them.
