Mal-A-Ket (1% keto, 2% acetic acid and 2% chlorhexidine) is now in a WET WIPE
as well as the new Shampoo.
Dermatophytosis is a serious problem facing veterinarians worldwide. Most cases
are seen in domestic cats.
Diagnosis is made by fungal culture, while some do hair and scale examinations,
as well, using potassium hydroxide or mineral oil preps. The classic macroconidia
known as arthroconidia are not seen in Trichophyton sp but rather in the more
common M canis or gypseum. Physical examination revealing fluorescence of hairs
is a vastly inferior diagnostic test as it is misread too often. Skin biopsy,
when spores are found in the hair follicle is pathognomonic though not seen
in all cases.
Cultures maybe obtained by plucking hairs in areas where there are lesions or
toothbrush cultures where not. A derm-duet with both dermatophyte media and
Sabourauds dextrose agar are best for culture at room temperature. Evaluation
for color change and colony formation are done daily over 2-4 weeks.
Topical therapy is important in all cases. It is especially important in very
young cases who are too small for accurate systemic dosing and other patients
who for various reasons can not undergo proper systemic therapy. The best topical
therapy known is lime sulfur (LIMEPLUS DIP, 97.8% sulfurated lime, DermaPet).
Other topical therapies that can be done concurrent with lime sulfur are various
formulations with ketoconazole (Mal-A-Ket. DermaPet), miconazole, acetic acid
(Mal-A-Ket), acetic and boric acids (MalAcetic, DermaPet), terbenafine (Lamisil)
and enilconazole. Enilconazole is illegal to use in the US and it is illegal
to use an EPA labeled product targeted for use in one species in another species.
Lime sulfur is by far the most effective and economical topical therapy. Positive
results as a result of topical therapy must be discounted since dermatophytosis
may self-cure in some patients.
Systemic therapies used for cats include itraconazole (5-10 mg/kg/qd) first
choice for cats), ketoconazole (10 mg/kg/QD) and fluconazole (5 mg/kg/BID in
dogs and cats) . Treatment must go on for 2 negative cultures.
Clipping the patient remains controversial. Advantages would be increased efficacy
of topical therapy, decreasing future environmental contamination revealing
hidden dermatologic manifestation of cutaneous disease (often useful to increase
client compliance). Disadvantages include environmental /facility contamination,
skin trauma and cosmetics.
Environmental treatments include bleaching (1:10) all contact items. Some people
use lime sulfur as well such as when using a carrier or cage to restrain the
patient. Professional steam cleaning (to assure properly high water temperatures)
of carpets and drapes is recommended.
It is beyond the scope of this presentation to discuss shelters. ALL cats are
sprayed with lime sulfur at admission and isolated from the general population.

Barbados, Commonwealth Veterinary Conference
YT opened the meeting with a lecture on 7 Steps to Treating Ears and Topical Therapy. For the next 4 days, the booth was almost totally ignored due to excellent beach and golf conditions. Lou Gotthelf, after ably presenting his Otology lectures and wet lab, and YT played the Barbados CC where, after a grueling match, Lou, with a hustling par on the 18th, eked out the back nince for a consolation, even though YT won the match.
Organized by Barbados resident vet, GUS READER, and his wife, DEBBIE, and their trusty associate, MICHELLE BELL,. the meeting ran seamlessly. There were no delays; despite fears of things slowing down and restless natives in the Caribbean, at least in Barbados all buses, events and programs stayed on schedule to the minute and the attitude was "can-do" with a smile.. BILL TOPLIFF (Medrx) had a good meeting, far exceeding his sales expectations. BOB COHEN (Spectrum Labs) was also quite pleased by the presence of new business. KERSTI SEKSAL (Past Pres, Australia VMA) and DIANE FRANK (Canada) presented Behavior. Animal Planet celebrity HARRY COOPER (Australia) and his wife JEANINE were most entertaining. And, then there was the world's best rum, both Cockspur and Mt Gay. The former gave out all the free VSOR rum during one of the free food and party evenings; the attitude of the crowd was much more exhilarating that night than the next morning. There were people from organized veterinary medicine from all over world notably Africa, India, Australia and the Caribbean. GRAHAM BROWN and CANDACE PHIPPS (Jamaica), TUNJI NASIR (Nigeria), SHARON SHEPARD (Trinidad), COLLIN BOYLE (St Vincent), JOSHUA DeSILVA (Guyana)just to name a few.
And, next year, we hope to see you at the Caribbean Veterinary 25th Biennial Conference November 6-9 in St Martin's.
MAUI, George Muller Derm Conference
JACK WHITE went to Hawaii while YT was in Barbados. Jack reported a good meeting attended by 185 vets including DICK McCARTHY and classmate, ED MOLESWORTH as well as our good friends, TESSY MATSUBARA and KAORU MATSUMOTO from Japan. The meeting was well organized by UC/Davis and laid back -the standard wear was shorts and aloha shirts.

Previously, we showed how our LIME PLUS Dip has 29% more lime sulfur than the new LymDip. The new LymDip is a much weaker formula that has not survived the test of time (neither for efficacy or stability). Despite their citing literature, there is not one refereed publication supporting the use of this substantially diluted product. There also is reason to believe that fragrances used in this product (the addition of which required the dilution from a 97.8% like LimePlus to 76.9% in LymDip) will cause contact irritation/dermatitis.
